The NTSB's final report on the January 2025 Washington DCA midair collision, which killed 67, cited FAA helicopter route placement and controller workload. On September 23, 2026 it issued its report on a 2024 Houston air tour helicopter crash into a radio tower that killed four, citing the pilot and a missed controller alert and noting weaker Part 91 tour oversight.

In late July 2026 a Supreme Court bench led by Chief Justice Surya Kant urged the Centre to consider standalone legislation defining 'digital arrest' as a specific offence, with stricter punishment. The government has said it is drafting legislation covering digital arrests and deepfakes. In July 2026 the CBI also arrested three people over a Rs 2.07 crore digital arrest fraud.

At least 24 children died after taking Coldrif cough syrup contaminated with diethylene glycol, with the maker's licence revoked and its founder in custody. A report updated in August 2026 describes failures in supply-chain control and inspections. The case recalls earlier deaths linked to Indian-made syrups abroad.

📍 Ahmedabad, Gujarat, India

The AAIB told the Supreme Court in July 2026 that a draft final report on the June 12, 2025 Ahmedabad crash, which killed 260 people, could be ready by October 2026. The bureau says the inquiry follows ICAO Annex 13 rules and that some evidence cannot be disclosed. Pilot bodies and a family have sought court-monitored oversight.

On July 29, 2026 Railway Minister Ashwini Vaishnaw told Parliament that Indian Railways recorded 2 consequential train accidents in 2026-27 so far. Kavach 4.0 covers 2,490 route km, with work under way on 21,937 more route km. Earlier reports note the system covered about 2,200 km in February 2026.

Shots were fired at actor Tiger Shroff's car near Pushpa Park in Juhu on 18 October. Police say he is unhurt. His driver and an autorickshaw driver were injured, and police formed 12 teams to trace the attackers. A gang claim online is not verified by police.

In early August 2026 the Supreme Court directed the RBI to issue, within four weeks, an SOP for temporary debit holds on mule accounts linked to cyber fraud, and asked States and UTs to set up money-restoration mechanisms. Reports say digital arrest complaints fell from 123,672 in 2024 to 16,377 by June 2026, with Rs 18.05 crore restored in 36,290 cases.

Maharashtra recorded 36,450 road accidents and 15,549 deaths in 2025, with two-wheeler riders about 57% of deaths and pedestrians 21%, according to a February 2026 report. The state declared 2026 the 'Save Two-Wheeler Riders and Pedestrians Year', with helmet enforcement, pedestrian infrastructure and radar interceptors.

Samruddhi Mahamarg recorded 185 accidents and 152 deaths in 2025, up from 137 and 126 in 2024, per a March 2026 report. A July 20, 2026 collision near Wardha killed six, and in September 2026 the state announced stricter speed enforcement, CCTV monitoring and faster emergency response for highways.

Mumbai's locals carry roughly 7.5 million passengers a day. In mid-July 2026 several incidents were reported, including a stick-wielding man at Kurla, a fight on a Parel-Ambarnath train and a dispute in a ladies' coach. Reports say GRP has 3,383 personnel against 4,185 sanctioned posts and RPF 5,225 against 7,042.
